Description

• Conduct thorough patient evaluations across inpatient or outpatient settings and develop individualized, goal-oriented treatment plans
• Deliver targeted physical therapy interventions addressing mobility deficits, functional limitations, strength impairments, and balance disorders
• Partner closely with physicians, nursing staff, case managers, and fellow rehabilitation professionals to drive optimal patient outcomes
• Provide meaningful education to patients and their families regarding therapeutic goals, home exercise programs, and safe discharge planning
• Maintain accurate and timely clinical documentation in compliance with facility standards and state and federal regulations
• Engage actively in interdisciplinary care conferences and team rounds to ensure continuity and coordination of care

Education

• Doctorate in Physical Therapy (DPT) or an equivalent degree earned from an accredited institution

Certifications

• Active state licensure as a Physical Therapist in good standing
• CPR certification preferred
