Description

• Conduct thorough patient evaluations across inpatient and outpatient settings and develop tailored, goal-driven treatment plans
• Deliver evidence-based physical therapy interventions targeting mobility, functional strength, balance, and overall independence
• Partner with an interdisciplinary team including physicians, nursing staff, case managers, and fellow rehabilitation specialists to drive optimal patient outcomes
• Provide clear and empowering education to patients and their families regarding therapy goals, home exercise programs, and safe discharge transitions
• Maintain accurate and timely clinical documentation in alignment with facility protocols and regulatory requirements
• Actively contribute to care team meetings, interdisciplinary rounds, and collaborative treatment planning discussions

Education

• Doctorate in Physical Therapy (DPT) or equivalent accredited degree

Certifications

• Active state licensure as a Physical Therapist in good standing
• CPR certification preferred
• Advanced or specialty certifications such as neurological or geriatric rehabilitation are welcomed but not required

Skills

• Prior clinical experience strongly preferred across any care setting
• Demonstrated ability to manage patients presenting with complex medical conditions and mobility challenges
