Description

• Conduct thorough evaluations of patients in both inpatient and outpatient settings, crafting personalized treatment plans tailored to their unique needs
• Deliver physical therapy treatments aimed at enhancing mobility, strength, balance, and overall functionality
• Work collaboratively with nursing staff, physicians, case managers, and fellow rehabilitation specialists to optimize patient outcomes
• Guide and educate patients and their families on therapeutic goals, the discharge process, and at-home exercise routines
• Document all aspects of patient evaluations, treatments, and progress according to hospital and regulatory standards
• Engage actively in patient care team rounds and interdisciplinary meetings to ensure a unified approach to treatment

Education

• A Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited educational institution

Certifications

• Current state licensure as a PT / Physical Therapist
• CPR certification is preferred
