Description

• Conduct thorough patient evaluations across inpatient and outpatient settings and develop personalized, goal-oriented treatment plans
• Deliver targeted physical therapy interventions designed to restore mobility, build strength, improve balance, and enhance overall functional performance
• Work alongside physicians, nurses, case managers, and fellow rehabilitation professionals to ensure cohesive, patient-centered care
• Guide patients and their families through therapeutic objectives, home exercise programming, and discharge readiness
• Maintain accurate, timely documentation of evaluations, sessions, and patient progress in alignment with regulatory and facility standards
• Engage in interdisciplinary rounds and care conferences to support smooth, coordinated care transitions

Education

• Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution

Certifications

• Active state licensure as a Physical Therapist in Arizona
• CPR certification preferred
• Specialty certifications in areas such as geriatrics, neurology, or orthopedics are welcomed but not required

Skills

• Prior clinical experience is strongly preferred
