Description

• Conduct thorough patient evaluations across inpatient or outpatient settings and develop individualized, goal-driven treatment plans
• Deliver targeted physical therapy interventions designed to improve strength, mobility, balance, and overall functional independence
• Work alongside physicians, nurses, case managers, and fellow rehabilitation specialists to drive meaningful patient outcomes
• Provide education to patients and their families around therapeutic goals, home exercise programs, and safe discharge planning
• Maintain accurate and timely clinical documentation in compliance with facility protocols and state and federal regulations
• Engage in interdisciplinary rounds and care conferences to ensure seamless, coordinated care across all touchpoints

Education

• Doctorate in Physical Therapy (DPT) or equivalent graduate degree from an accredited institution

Certifications

• Active state licensure as a Physical Therapist in good standing
• CPR certification preferred
• Advanced or specialty certifications in areas such as geriatrics or neurology are welcomed, though not required

Skills

• Prior clinical experience is strongly preferred
