Description

• Conduct thorough patient evaluations across inpatient or outpatient settings and develop individualized, goal-driven treatment plans
• Deliver targeted physical therapy interventions addressing mobility, muscular strength, balance, coordination, and functional independence
• Work alongside physicians, nurses, case managers, and fellow rehabilitation specialists to ensure holistic, outcome-focused care
• Guide patients and their families through therapeutic objectives, home exercise programs, and discharge planning
• Maintain accurate, timely documentation of assessments, interventions, and patient progress in alignment with facility and regulatory standards
• Engage actively in interdisciplinary rounds and care conferences to promote seamless, coordinated treatment across the continuum

Education

• Doctorate in Physical Therapy (DPT) or equivalent degree from an accredited institution

Certifications

• Active state licensure as a Physical Therapist in good standing
• CPR certification preferred
• Advanced or specialty certifications (such as neurological or geriatric PT) are welcome, though not required

Skills

• Prior clinical experience strongly preferred
• Demonstrated ability to manage patients presenting with complex medical histories and mobility challenges
