Description

• Conduct thorough patient evaluations across inpatient or outpatient settings and develop personalized, goal-driven treatment plans
• Deliver targeted physical therapy interventions addressing mobility deficits, functional limitations, strength, and balance
• Partner closely with interdisciplinary care teams — including physicians, nursing staff, case managers, and fellow rehab clinicians — to drive optimal patient outcomes
• Guide patients and their families through therapeutic goals, safe discharge planning, and home exercise programs
• Maintain accurate, timely documentation of evaluations, interventions, and patient progress in alignment with facility protocols and regulatory standards
• Actively contribute to team rounds and interdisciplinary care conferences to support seamless, coordinated care delivery

Education

• Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution

Certifications

• Valid state licensure as a Physical Therapist (or eligibility to obtain licensure in Arizona)
• CPR certification preferred
• Specialty certifications in areas such as geriatrics, neurology, or orthopedics are welcomed but not required
