Description

• Conduct evaluations for patients in both inpatient and outpatient environments, creating tailored treatment protocols
• Implement physical therapy interventions focusing on enhancing mobility, strength, balance, and overall functionality
• Work closely with nurses, physicians, case managers, and fellow rehabilitation specialists to maximize patient outcomes
• Provide education to patients and their families on therapeutic objectives, discharge plans, and home exercise routines
• Maintain accurate documentation of evaluations, treatments, and progress in line with facility policies and regulatory requirements
• Engage in care team rounds and interdisciplinary meetings to ensure cohesive care delivery

Education

• Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution

Certifications

• State licensure as a Physical Therapist
• CPR certification is a plus
