Description

• Conduct thorough patient evaluations across inpatient or outpatient settings and develop individualized, goal-oriented treatment plans
• Deliver hands-on physical therapy interventions targeting strength, balance, mobility, and overall functional performance
• Partner with a multidisciplinary care team — including physicians, nurses, case managers, and fellow rehabilitation specialists — to drive optimal patient outcomes
• Guide patients and their families through therapeutic goals, home exercise programs, and safe discharge planning
• Maintain accurate, timely clinical documentation in alignment with facility protocols and regulatory standards
• Engage in interdisciplinary care rounds and team conferences to ensure continuity and coordination of care

Education

• Doctorate in Physical Therapy (DPT) or equivalent degree from an accredited institution

Certifications

• Active state licensure as a Physical Therapist in California
• CPR certification preferred
• Specialty certifications in areas such as geriatrics, neurology, or orthopedics are welcomed but not required
