Description

• Conduct thorough patient evaluations in both inpatient and outpatient environments and develop individualized, goal-oriented care plans
• Deliver targeted physical therapy interventions addressing mobility, strength, balance, neuromuscular function, and daily living activities
• Partner with a multidisciplinary team including physicians, nurses, occupational therapists, and case managers to drive holistic patient outcomes
• Guide patients and their families through therapeutic goals, exercise programs, and discharge planning with clarity and compassion
• Maintain thorough and timely documentation of evaluations, interventions, and patient progress in compliance with facility and state regulatory standards
• Actively participate in interdisciplinary rounds, care conferences, and quality improvement initiatives

Education

• Doctorate in Physical Therapy (DPT) or equivalent degree from a regionally accredited physical therapy program

Certifications

• Active Physical Therapist licensure in the state of California (or eligibility to obtain)
• CPR / BLS certification preferred
• Specialty certifications in areas such as neurological rehab, orthopedics, or geriatrics are welcomed but not required
