Description

• Perform comprehensive patient evaluations in inpatient or outpatient settings and develop individualized, goal-oriented treatment plans
• Deliver targeted physical therapy interventions addressing mobility, strength, balance, coordination, and functional independence
• Partner with interdisciplinary care teams including physicians, nurses, case managers, and fellow rehabilitation professionals to drive optimal patient outcomes
• Guide patients and their families through therapeutic goals, home exercise programs, and safe discharge transitions
• Maintain thorough and timely documentation of evaluations, progress notes, and treatment records in compliance with facility and regulatory standards
• Engage actively in team rounds and interdisciplinary care conferences to support coordinated, patient-centered care

Education

• Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution

Certifications

• Active state licensure as a Physical Therapist in good standing
• CPR certification preferred
