Description

• Conduct thorough patient evaluations across inpatient or outpatient settings and develop individualized, goal-oriented treatment plans
• Deliver physical therapy interventions targeting mobility, functional strength, balance, and overall physical performance
• Partner with interdisciplinary care teams — including physicians, nurses, case managers, and fellow rehab professionals — to drive optimal patient outcomes
• Guide patients and their families through therapeutic goals, home exercise programming, and discharge preparation
• Maintain accurate and timely documentation of evaluations, treatment sessions, and clinical progress in alignment with facility protocols and regulatory standards
• Actively engage in interdisciplinary rounds and care conferences to ensure seamless, coordinated patient care

Education

• Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution

Certifications

• Active state licensure as a Physical Therapist in California
• CPR certification preferred
• Specialty certifications in areas such as geriatrics, neurology, or orthopedics are welcomed but not required

Skills

• Prior clinical experience is strongly preferred
• Demonstrated ability to manage patients presenting with complex medical and mobility challenges
• Excellent interpersonal, communication, and collaborative teamwork skills
• Proficiency with electronic health records and digital clinical documentation platforms
