Position Overview

This role is responsible for the comprehensive coordination of an Acute Rehabilitation Program, focusing on program development, marketing, and operational efficiency. Acting as a key liaison among physicians, clinical staff, external facilities, and payers, the coordinator fosters referral growth and ensures smooth patient transitions. Communication with nursing and operational leadership, as well as medical directors, is essential to report on goals, market expansion, regulatory compliance, and operational improvements. The coordinator also serves as a mentor and clinical resource to the rehabilitation team.

Key Responsibilities

• Maintain a safe and clean work environment, adhering to unit safety and infection control standards.
• Coordinate admissions, transfers, and discharges in collaboration with attending staff, nursing, and external facilities, ensuring timely and accurate documentation.
• Support leadership in budget forecasting and variance analysis; assist with staffing, scheduling, supplies, and equipment needs.
• Participate in performance improvement initiatives, promoting program growth through community outreach and marketing efforts.
• Conduct onsite assessments for potential external referrals.
• Manage insurance pre-authorizations and ongoing approvals; track utilization and departmental statistics.
• Ensure compliance with federal, state, and accreditation regulations, maintaining awareness of legal and safety considerations.
• Engage in weekly interdisciplinary patient care conferences.
• Perform additional duties as assigned by management.

Qualifications

• Minimum of three years progressive leadership or supervisory experience preferred.
• At least two years of clinical practice in an acute care setting required.
• Working knowledge of Medicare admission and reimbursement criteria.
• Experience in Utilization Management or Discharge Planning is a plus.
• Proficiency with standard office equipment and computer software (MS Outlook, Excel, Word, PowerPoint, internet).
• Strong communication, problem-solving, interpersonal, organizational, and customer service skills.

Education

• Graduate of an accredited program in Physical Therapy, Occupational Therapy, Registered Nursing, or Speech Therapy.

Licenses and Certifications

• Current license in Physical Therapy, Occupational Therapy, Registered Nursing, or Speech Therapy.
• Valid driver’s license and auto insurance.
• Current BLS certification from the American Heart Association Healthcare Provider.
• Maintain any unit-specific certifications as required.

All candidates must successfully complete a physical evaluation, drug screening, and background checks prior to employment. This position is located in a regional healthcare setting within the Pacific Southwest region of the United States.
