Description

• Perform thorough patient evaluations across inpatient and outpatient settings and develop individualized, goal-driven treatment plans
• Deliver physical therapy interventions targeting mobility restoration, functional strength, postural balance, and independence
• Work in close partnership with physicians, nurses, case managers, and fellow rehabilitation specialists to drive optimal patient outcomes
• Guide patients and their families through therapeutic goals, home exercise programs, and discharge planning strategies
• Maintain accurate and timely documentation of evaluations, interventions, and progress notes in alignment with facility and regulatory standards
• Participate actively in interdisciplinary team rounds and care conferences to ensure seamless, coordinated treatment

Education

• Doctorate in Physical Therapy (DPT) or equivalent degree from an accredited institution

Certifications

• Active state licensure as a Physical Therapist in California
• Current CPR certification preferred
• Advanced or specialty certifications such as neurological or geriatric rehab are welcomed, though not required

• Prior clinical experience is strongly preferred
