Position Overview

This role is dedicated to delivering clinical and palliative care services within hospital or homecare environments. The practitioner supports the assessment, development, implementation, monitoring, and evaluation of clinical programs and policies. They actively contribute to quality improvement initiatives and collaborate closely with physicians and healthcare teams to optimize patient outcomes and resource management.

Key Responsibilities

• Maintain a safe and clean work environment, adhering to unit safety and infection control protocols.
• Provide clinical services in hospital settings, monitoring patient outcomes and identifying barriers to care.
• Educate physicians, inpatient staff, and the community on hospice and end-of-life care services.
• Participate in performance improvement programs and assist in developing clinical policies, procedures, and monitoring tools.
• Assess patient health status through histories and examinations, referring patients to alternate care when necessary.
• Collaborate with infection control teams on reporting, screening, and prevention programs for infectious diseases.
• Prepare and present monthly reports and statistics to management; attend relevant meetings and workshops.
• Work as an integral member of a multidisciplinary team to enhance clinical and financial outcomes.

Qualifications

• Minimum of 2 years experience as an adult Nurse Practitioner, or at least 5 years of acute care RN experience with some NP clinical experience.
• Completion of all Nurse Practitioner clinical requirements.
• Proficient with personal computers and excellent communication and customer service skills.
• Bilingual in English and Spanish is strongly preferred.

Education

• Graduate of an accredited nursing school.
• Master of Science in Nursing (MSN) degree required.

Licenses and Certifications

• Current RN and Nurse Practitioner licenses valid in the region.
• Current Basic Life Support (BLS) certification from the American Heart Association.
