Description

• Conduct thorough patient evaluations across inpatient or outpatient settings and develop individualized, goal-oriented treatment plans
• Deliver physical therapy interventions targeting improvements in mobility, functional strength, balance, and independence
• Partner with a multidisciplinary team including physicians, nurses, case managers, and rehabilitation specialists to deliver seamless, coordinated care
• Guide patients and their families through therapeutic goals, home exercise programs, and discharge readiness
• Maintain accurate, timely documentation of evaluations, session notes, and patient progress in compliance with facility and regulatory standards
• Actively contribute to interdisciplinary rounds and care conferences to support whole-patient outcomes

Education

• Doctorate in Physical Therapy (DPT) or equivalent degree from an accredited institution

Certifications

• Active state licensure as a Physical Therapist in California
• CPR certification preferred
• Specialty certifications in areas such as geriatrics, neurology, or orthopedics are welcomed but not required
