Your Day-to-Day Responsibilities

• Performing thorough, patient-centered assessments and developing individualized, evidence-based care plans
• Diagnosing and managing acute, episodic, and chronic health conditions, independently or in collaboration with supervising physicians as applicable
• Prescribing medications and therapeutic interventions within your credentialed scope of practice
• Educating patients and their support systems on disease prevention, health maintenance, and self-management strategies
• Following up with patients through in-person visits and telehealth platforms to ensure continuity of care
• Engaging actively with interdisciplinary care teams to deliver seamless, whole-person healthcare
• Maintaining precise, timely documentation across electronic medical record platforms

Education Requirements

• Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP) from an accredited institution
• Completion of an accredited Nurse Practitioner training program

Certifications Required

• Active Nurse Practitioner licensure in the state of California
