Description

• Conduct thorough patient evaluations in inpatient or outpatient settings and develop tailored, goal-driven treatment plans
• Deliver physical therapy interventions targeting strength, mobility, balance, coordination, and overall functional independence
• Work in close partnership with physicians, nursing staff, case managers, and fellow rehabilitation specialists to drive optimal patient outcomes
• Guide patients and their families through therapeutic goals, home exercise programming, and safe discharge planning
• Maintain thorough and timely documentation of evaluations, session notes, and progress in compliance with facility protocols and regulatory standards
• Take an active role in interdisciplinary team rounds and care conferences to ensure seamless, well-coordinated treatment

Education

• Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution

Certifications

• Active state licensure as a Physical Therapist in California
• CPR certification preferred
• Specialty certifications in areas such as geriatrics, neurology, or orthopedics are welcomed but not mandatory

Skills

• Prior clinical experience strongly preferred
• Demonstrated ability to manage patients with complex or multifaceted medical and mobility challenges
