Role Responsibilities

• Deliver comprehensive, relationship-centered care to patients spanning all age groups, from young children to older adults.
• Manage chronic disease, acute illness, preventive screenings, and behavioral health needs using a whole-person framework.
• Perform physical examinations, order and interpret diagnostics, and craft individualized care plans tailored to each patient's goals.
• Work in close partnership with care coordinators, behavioral health clinicians, and specialists to ensure seamless, integrated care delivery.
• Engage actively in quality improvement efforts that drive better patient outcomes and a more efficient practice environment.
• Maintain thorough and timely documentation within the electronic medical record (EMR) system.
• Extend your reach to patients through both in-person visits and telehealth appointments as appropriate.

Education Requirements

• MD or DO degree from an accredited institution.
• Successful completion of an accredited Family Medicine residency program.

Certifications

• Board Certification in Family Medicine (or active pursuit of certification).
• Active California medical license or eligibility to obtain one.
• BLS and ACLS certifications preferred.
