Description

• Perform thorough patient evaluations across inpatient or outpatient settings and develop personalized, goal-driven treatment plans
• Deliver physical therapy interventions targeting strength, mobility, coordination, and overall functional performance
• Partner with a multidisciplinary team — including physicians, nurses, case managers, and fellow rehab specialists — to drive optimal patient outcomes
• Guide patients and their families through therapeutic goals, home exercise programs, and discharge planning with clarity and compassion
• Maintain accurate, timely documentation of evaluations, treatment sessions, and patient progress in compliance with regulatory and facility standards
• Engage actively in interdisciplinary rounds and care conferences to ensure seamless, coordinated treatment across the continuum of care

Education

• Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution

Certifications

• Active state licensure as a Physical Therapist in California
• CPR certification preferred
• Specialty certifications in areas such as geriatrics, neurology, or orthopedics are welcomed but not required

Skills

• Prior clinical experience is strongly preferred
• Demonstrated ability to manage patients with complex medical presentations and mobility challenges
• Excellent interpersonal and collaborative communication skills
• Proficiency with electronic health records and digital documentation platforms
