Position Overview

This role is responsible for the comprehensive coordination of the Acute Rehabilitation Program, including program development, marketing, and ensuring operational efficiency and effectiveness. Acting as a key liaison among physicians, staff, external facilities, and payers, the coordinator fosters referral growth and communicates progress and recommendations to leadership and medical staff. This position also serves as a mentor and clinical resource for the rehabilitation team.

Key Responsibilities

• Maintain a safe and clean work environment, adhering to unit safety and infection control standards.
• Coordinate admissions, transfers, and discharges in collaboration with attending staff, nursing, and external facilities, ensuring timely and accurate documentation.
• Support leadership in budget forecasting and variance analysis, reporting performance metrics to executive councils as needed.
• Participate in performance improvement initiatives, community outreach, and marketing efforts to promote program growth.
• Conduct onsite assessments for potential patient admissions from external referrals.
• Manage insurance pre-authorizations and ongoing approvals, tracking utilization and statistical data.
• Ensure compliance with federal, state, and accreditation regulations, maintaining awareness of legal and safety considerations.
• Engage in weekly interdisciplinary patient care conferences.
• Perform additional duties as assigned by supervisors.

Qualifications

• Minimum of three years progressive leadership or supervisory experience preferred.
• At least two years of clinical practice in an acute care setting required.
• Working knowledge of Medicare admission and reimbursement criteria.
• Experience in Utilization Management or Discharge Planning is a plus.
• Proficiency with standard office equipment and software including MS Outlook, Excel, Word, and PowerPoint.
• Strong communication, problem-solving, interpersonal, organizational, and customer service skills.

Education

• Graduate of an accredited program in Physical Therapy, Occupational Therapy, Registered Nursing, or Speech Therapy.

Licenses and Certifications

• Current state license in Physical Therapy, Occupational Therapy, Registered Nursing, or Speech Therapy.
• Valid driver’s license and auto insurance.
• Current BLS certification from the American Heart Association Healthcare Provider.
• Maintain any unit-specific certifications as required.
