Position Overview

This role is dedicated to delivering clinical and palliative care services within hospital or homecare environments. The practitioner supports the assessment, development, implementation, monitoring, and evaluation of clinical programs and policies. They actively contribute to quality improvement initiatives and collaborate closely with physicians and healthcare teams to optimize patient outcomes and resource management.

Key Responsibilities

• Maintain a safe and clean work environment, adhering to unit safety and infection control protocols.
• Provide clinical services in hospital settings, including patient health assessments and examinations.
• Identify and address barriers to care by working with interdisciplinary teams; educate staff and community on hospice and end-of-life care.
• Oversee clinical service development and evaluation, including policy and procedure formulation.
• Coordinate with infection control departments on disease prevention and reporting.
• Prepare and present monthly reports and statistics to management; participate in relevant meetings and workshops.
• Continuously improve quality of care and clinical services.
• Collaborate effectively with medical directors, physicians, and healthcare professionals to ensure efficient service delivery.

Qualifications

• Minimum of 2 years experience as an adult Nurse Practitioner, or alternatively, at least 5 years of RN acute care experience with some NP experience.
• Completion of all Nurse Practitioner clinical requirements.
• Proficient with personal computers.
• Excellent communication and customer service skills.
• Bilingual in English and Spanish is strongly preferred.

Education

• Graduate of an accredited nursing school.
• Master of Science in Nursing (MSN) degree required.

Licenses and Certifications

• Current Registered Nurse (RN) license.
• Current Nurse Practitioner license.
• Basic Life Support (BLS) certification from the American Heart Association required upon hire.
