Description

• Conduct thorough patient evaluations in inpatient or outpatient environments and develop individualized, goal-driven treatment plans
• Deliver targeted physical therapy interventions designed to restore mobility, functional strength, and balance
• Work closely alongside physicians, nurses, case managers, and interdisciplinary rehab specialists to drive optimal patient outcomes
• Provide patient and family education covering therapeutic goals, home exercise programs, and discharge readiness
• Maintain accurate and timely documentation of evaluations, sessions, and clinical progress per facility and regulatory standards
• Actively participate in care team rounds and interdisciplinary conferences to support seamless, coordinated care delivery

Education

• Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution

Certifications

• Active state licensure as a Physical Therapist in California
• CPR certification preferred
• Specialty certifications in areas such as geriatrics, neurology, or orthopedics are welcomed but not required

Skills

• Prior clinical experience is strongly preferred
• Demonstrated ability to manage patients presenting with complex medical and mobility challenges
• Exceptional interpersonal and team communication skills
• Proficiency with electronic health records and digital clinical documentation platforms
