Description

• Perform thorough patient evaluations across inpatient and outpatient settings and develop individualized, goal-driven treatment plans
• Deliver evidence-based physical therapy interventions targeting functional mobility, muscular strength, coordination, and balance
• Work in close partnership with interdisciplinary care teams including physicians, nurses, occupational therapists, and case managers
• Guide patients and their support networks through therapeutic goals, home exercise programming, and discharge readiness
• Maintain accurate, timely documentation of assessments, interventions, and patient progress per regulatory and facility standards
• Contribute actively to interdisciplinary rounds and care conferences to promote seamless, coordinated patient outcomes

Education

• Doctorate of Physical Therapy (DPT) or equivalent from an accredited institution

Certifications

• Active Physical Therapist licensure in the state of California
• CPR certification preferred
