Description

• Perform comprehensive patient evaluations across inpatient or outpatient settings and develop individualized, goal-oriented treatment plans
• Deliver targeted physical therapy interventions addressing mobility, functional strength, balance, and independence
• Partner closely with physicians, nursing staff, case managers, and fellow rehabilitation professionals to drive optimal patient outcomes
• Provide meaningful education to patients and their families regarding therapeutic goals, home exercise programs, and safe discharge planning
• Maintain accurate and thorough clinical documentation in compliance with all facility protocols and regulatory standards
• Actively contribute to interdisciplinary care conferences and team rounds to ensure seamless, coordinated care delivery

Education

• Doctorate in Physical Therapy (DPT) or equivalent degree from an accredited institution

Certifications

• Active state licensure as a Physical Therapist in California
• CPR certification preferred
