Description

• Conduct thorough patient evaluations in inpatient or outpatient settings and develop individualized, goal-oriented treatment plans
• Deliver targeted physical therapy interventions addressing mobility, strength, coordination, balance, and functional independence
• Partner with physicians, nursing staff, case managers, and interdisciplinary rehab colleagues to drive optimal patient outcomes
• Guide patients and their families through therapeutic goals, home exercise programs, and safe discharge planning
• Maintain detailed, accurate clinical documentation in alignment with facility standards and regulatory requirements
• Actively participate in care conferences and interdisciplinary rounds to ensure seamless, coordinated care delivery

Education

• Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution

Certifications

• Active California state licensure as a Physical Therapist
• CPR certification preferred
• Specialty certifications in areas such as neurology, geriatrics, or orthopedics are welcomed but not required

Skills

• Prior clinical experience strongly preferred
• Demonstrated ability to manage patients with complex, multifaceted medical and mobility needs
