Description

• Conduct thorough evaluations across inpatient and outpatient settings to assess patient needs and establish individualized rehabilitation plans
• Deliver targeted physical therapy interventions addressing mobility deficits, strength impairments, balance disorders, and functional limitations
• Work in close coordination with interdisciplinary teams including physicians, nurses, occupational therapists, and case managers to drive optimal patient outcomes
• Provide patient and caregiver education around home exercise programs, therapeutic goals, and safe discharge planning
• Maintain accurate, timely documentation of evaluations, session notes, and patient progress in compliance with facility and state regulations
• Engage actively in interdisciplinary rounds and care conferences to ensure seamless, patient-centered care delivery

Education

• Doctorate in Physical Therapy (DPT) or equivalent from a nationally accredited program

Certifications

• Active Physical Therapist licensure in the state of California
• CPR certification preferred
• Advanced or specialty certifications such as neurological rehabilitation or geriatric PT are welcomed but not required

Skills

• Prior clinical experience in inpatient or outpatient rehabilitation settings is strongly preferred
• Demonstrated ability to manage patients presenting with complex, multifaceted conditions
• Exceptional interpersonal communication and collaborative teamwork abilities
• Proficiency with electronic health records and digital clinical documentation platforms
