Description

• Perform comprehensive patient evaluations across inpatient or outpatient settings and develop individualized, goal-driven treatment plans
• Deliver hands-on physical therapy interventions targeting strength, balance, mobility, and functional independence
• Work in close partnership with physicians, nurses, case managers, and rehabilitation specialists to achieve the best possible patient outcomes
• Guide patients and their families through therapeutic goals, safe discharge transitions, and home exercise programming
• Maintain thorough and timely documentation of evaluations, interventions, and clinical progress in compliance with facility and regulatory standards
• Actively participate in interdisciplinary rounds and care conferences to promote coordinated, patient-centered care

Education

• Doctorate in Physical Therapy (DPT) or an equivalent degree from a regionally accredited institution

Certifications

• Active state licensure as a Physical Therapist in California
• CPR certification preferred
• Specialty certifications in areas such as neurology, geriatrics, or orthopedics are welcome but not required

Skills

• Prior clinical experience is strongly preferred
• Demonstrated ability to manage patients presenting with complex medical and mobility challenges
• Exceptional interpersonal and team communication skills
