Description

• Conduct thorough patient evaluations in inpatient or outpatient settings and develop individualized, goal-driven treatment plans
• Deliver evidence-based physical therapy interventions targeting mobility, strength, balance, coordination, and functional independence
• Work closely with physicians, nurses, case managers, and fellow rehabilitation specialists to drive optimal patient outcomes
• Guide patients and their families through therapeutic goals, home exercise programming, and discharge preparation
• Maintain accurate, timely documentation of evaluations, interventions, and patient progress per facility and regulatory standards
• Actively engage in interdisciplinary care conferences and rounds to support seamless, coordinated care delivery

Education

• Doctorate in Physical Therapy (DPT) or equivalent accredited degree

Certifications

• Active California state licensure as a Physical Therapist
• CPR certification preferred
• Specialty certifications in areas such as geriatrics, neurology, or orthopedics are welcomed but not mandatory

Skills

• Prior clinical experience is strongly preferred
• Demonstrated ability to manage patients presenting with complex medical and mobility challenges
• Exceptional interpersonal and communication skills with a collaborative mindset
• Confident using electronic health record systems and digital clinical documentation platforms
