Description

• Conduct thorough patient evaluations across inpatient and outpatient settings and develop individualized, goal-oriented treatment plans
• Deliver targeted physical therapy interventions addressing mobility, neuromuscular function, balance, strength, and overall physical independence
• Partner closely with physicians, nursing staff, case managers, and fellow rehabilitation professionals to ensure cohesive, patient-centered care
• Guide patients and their families through therapeutic goals, home exercise programming, and safe discharge transitions
• Maintain accurate, timely, and compliant clinical documentation in alignment with facility protocols and state regulations
• Actively engage in interdisciplinary rounds, care conferences, and team discussions to drive optimal recovery outcomes

Education

• Doctorate in Physical Therapy (DPT) or equivalent graduate degree from an accredited institution

Certifications

• Active Physical Therapist licensure in the state of California
• CPR certification preferred
