Description

• Perform thorough patient evaluations across inpatient or outpatient settings and build individualized, goal-driven treatment plans
• Deliver targeted physical therapy interventions designed to rebuild mobility, strength, coordination, and functional independence
• Work closely with a multidisciplinary team that includes physicians, nurses, case managers, and fellow rehabilitation professionals to ensure cohesive, patient-centered care
• Guide patients and their families through therapeutic goals, home exercise programming, and discharge planning with clarity and compassion
• Maintain precise, timely documentation of evaluations, progress notes, and treatment records in compliance with facility and regulatory standards
• Actively engage in interdisciplinary care conferences and team rounds to contribute your clinical insight and advocacy for patient outcomes

Education

• Doctorate in Physical Therapy (DPT) or equivalent degree from a regionally accredited institution

Certifications

• Active Physical Therapist licensure in the state of Colorado (or eligibility to obtain)
• CPR certification preferred
• Specialty certifications in areas such as geriatrics, orthopedics, or neurology are welcomed but not required

Skills

• Prior clinical experience is strongly preferred
• Demonstrated ability to manage patients presenting with complex medical histories and functional limitations
• Excellent interpersonal communication and a natural instinct for teamwork
