Description

• Conduct thorough patient evaluations across inpatient and outpatient environments and build individualized, goal-oriented care plans
• Deliver targeted PT interventions addressing mobility deficits, strength restoration, balance impairment, and functional independence
• Work hand-in-hand with physicians, nursing staff, case managers, and fellow rehab professionals to drive the best possible outcomes
• Counsel patients and their families regarding therapeutic objectives, safe discharge strategies, and at-home exercise programming
• Maintain accurate and timely clinical documentation aligned with facility protocols and state and federal regulatory standards
• Contribute actively to interdisciplinary rounds, team conferences, and care coordination efforts across the continuum

Education

• Doctorate of Physical Therapy (DPT) or equivalent qualifying degree from a fully accredited physical therapy program

Certifications

• Active state licensure as a Physical Therapist in good standing
• CPR certification preferred
• Advanced or specialty certifications such as neurological rehab or geriatric PT are welcomed, though not required
