Description

• Conduct thorough patient evaluations across inpatient or outpatient settings and build individualized, goal-oriented treatment plans
• Deliver physical therapy interventions targeting strength, mobility, balance, coordination, and functional independence
• Work alongside physicians, nurses, case managers, and fellow rehab specialists to deliver seamless, patient-centered care
• Guide patients and their families through therapeutic goals, home exercise programs, and discharge planning with clarity and empathy
• Maintain accurate, timely documentation of evaluations, progress notes, and treatment records in alignment with regulatory and facility standards
• Engage actively in interdisciplinary care conferences and rounds to ensure continuity and quality of care across the team

Education

• Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution

Certifications

• Active state licensure as a Physical Therapist
• CPR certification preferred
• Specialty certifications such as neurological or geriatric rehabilitation are welcomed but not required

Skills

• Prior clinical experience is strongly preferred
• Demonstrated ability to manage patients with complex medical histories and mobility challenges
• Exceptional interpersonal and communication skills with a team-first mindset
• Proficiency with electronic health records and digital clinical documentation platforms
