Description

• Conduct thorough evaluations of patients in both inpatient and outpatient settings and develop customized treatment plans
• Implement physical therapy interventions designed to enhance mobility, strength, balance, and overall function
• Partner with nurses, physicians, case managers, and other rehabilitation professionals to ensure optimal patient outcomes
• Guide patients and their families on therapeutic objectives, discharge planning, and at-home exercise regimes
• Maintain comprehensive documentation of evaluations, treatments, and patient progress, adhering to facility and regulatory standards
• Engage in multidisciplinary care team meetings and conferences to ensure seamless patient support

Education

• Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution

Certifications

• The requisite state licensure as a Physical Therapist
• CPR certification preferred
