Description

• Conduct evaluations for patients in inpatient or outpatient settings and develop personalized treatment plans
• Implement physical therapy interventions aimed at improving mobility, strength, balance, and overall functionality
• Work collaboratively with nurses, physicians, case managers, and other rehabilitation specialists to enhance patient outcomes
• Guide patients and their families on therapeutic goals, discharge plans, and at-home exercises
• Keep thorough records of evaluations, treatment protocols, and patient progress in alignment with facility and regulatory standards
• Engage in care team discussions and interdisciplinary meetings to ensure well-coordinated care

Education

• Doctorate in Physical Therapy (DPT) or equivalent from an accredited institution

Certifications

• Valid state licensure as a Physical Therapist
• CPR certification is preferred
• Additional certifications (e.g., neurological, geriatric) are advantageous but not mandatory

• Previous experience is strongly preferred
