Education Requirements

• MD or DO degree from an accredited and recognized medical institution.
• Successful completion of an accredited Family Medicine residency program.

Certifications

• Board Certification in Family Medicine (or active board eligibility).
• Active Florida medical license or demonstrated eligibility to obtain one.
• BLS and ACLS certifications preferred.
