Description

• Perform comprehensive patient evaluations across inpatient and/or outpatient settings and develop individualized, goal-driven treatment plans
• Deliver physical therapy interventions targeting mobility, muscular strength, coordination, balance, and overall functional independence
• Partner closely with interdisciplinary care teams — including physicians, nurses, and case managers — to drive optimized patient outcomes
• Provide thorough education to patients and their families regarding treatment goals, safe home exercise programs, and discharge expectations
• Maintain accurate, timely clinical documentation in compliance with facility protocols and regulatory standards
• Actively engage in care team huddles and interdisciplinary meetings to promote seamless, coordinated patient care

Education

• Doctorate in Physical Therapy (DPT) or equivalent degree from an accredited institution

Certifications

• Active state licensure as a Physical Therapist in good standing
• CPR certification preferred
• Specialized credentials in areas such as geriatrics, neurology, or orthopedics are welcomed but not required

Skills

• Prior clinical experience is strongly preferred, though motivated newer graduates are encouraged to apply
• Demonstrated ability to manage patients with complex medical histories and multifaceted mobility challenges
