We are seeking a compassionate and skilled Registered Nurse (RN) Care Coordinator to join a dedicated healthcare team serving the Midwest region. This role focuses on delivering comprehensive, patient-centered care to individuals managing multiple chronic and preventive health conditions.

Key Responsibilities

• Provide holistic support and counseling to patients, addressing barriers to care and promoting medication adherence, healthy lifestyle choices, and self-management strategies.
• Monitor high-risk patients through monthly outcome reports, collaborating with providers to ensure appropriate follow-up and care adjustments.
• Conduct nursing assessments and medical triage for acute issues and medication side effects, referring or monitoring as needed.
• Educate patients, families, staff, and community partners on chronic disease management and preventive screenings in an organized and effective manner.
• Serve as a liaison to connect patients with community support services, financial assistance, and entitlement resources.
• Manage referrals and follow-up for specialty medical, mental health, substance abuse, and community services.
• Utilize electronic medical records in compliance with established guidelines and maintain accurate documentation of billable procedures.
• Collaborate as an interdisciplinary team member to ensure high-quality care delivery.
• Perform direct nursing interventions such as venipuncture, wound care, and vaccinations as required.
• Maintain confidentiality and adhere to all regulatory and compliance standards, including HIPAA.

Education and Experience

• Graduate of an accredited nursing program with current licensure as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) authorized to practice in the region.
• Minimum of three years clinical experience as an RN or LPN, with at least two years in a similar care coordination role preferred.
• Current Basic Life Support (BLS) certification by the American Heart Association.

• Completion of Mandatory Reporter training within six months of hire.
• Commitment to ongoing education and compliance with federal, state, and professional licensure standards.

• Ability to maintain confidentiality and work cooperatively with patients, staff, and community partners.
• Physical ability to perform nursing duties and maintain regular attendance.
