The Oncology Nurse Navigator plays a vital role in guiding cancer patients through every stage of their journey—from diagnosis to treatment, recovery, and survivorship. This position involves delivering compassionate patient care through comprehensive nursing processes including assessment, planning, implementation, and evaluation. The nurse navigator also supports physicians, nurse practitioners, and physician assistants by facilitating patient education and care coordination.

Key Responsibilities

• Serve as the primary point of contact for patients and families following an initial cancer diagnosis.
• Track patient progress through testing and treatment, identifying needs and connecting patients with appropriate resources.
• Coordinate care across the continuum, ensuring seamless communication among care team members.
• Provide education and support to patients and families, collaborating closely with multidisciplinary teams.
• Assist in developing and maintaining policies and procedures related to oncology care.
• Advocate for patients by addressing barriers to care and offering counseling.
• Monitor patient outcomes, review results with providers, and initiate necessary follow-up actions.
• Facilitate patient referrals and insurance authorizations.
• Demonstrate empathy and deliver exceptional customer service to diverse patient populations.
• Work collaboratively with cancer care teams and other departments to schedule follow-ups and testing.
• Maintain knowledge of departmental protocols and access resources effectively.
• Participate in relevant meetings and committees to support oncology services.

Education and Experience

• Graduate of an accredited nursing program with current Registered Nurse licensure (RN) or Licensed Practical Nurse (LPN) licensure valid in the region.
• Minimum of 3 years clinical nursing experience required.
• Oncology Nursing Certification (ONC) preferred.
• Experience in oncology, chemotherapy administration, and cancer patient care highly desirable.
• Proficiency with electronic health records, preferably Epic, is preferred.
