Join a growing Home Sleep and Medical Department serving the Midwest region, where patient care and service excellence are top priorities. As a Licensed Practical Nurse (LPN), you will play a vital role in delivering compassionate nursing care through assessment, planning, implementation, and evaluation. You will work closely under the supervision of a Registered Nurse or physician, assisting with medication administration and patient teaching while adhering to professional nursing standards.

Key Responsibilities

• Accurately transcribe and communicate oral and written orders.
• Provide nursing care following the interdisciplinary plan, including medication administration and monitoring.
• Prepare and administer oral and injectable medications, assist with non-medicated IV fluids, and monitor IV infusions.
• Obtain patient history and perform ongoing physical and psychosocial reassessments.
• Implement clinical procedures such as urinary catheter insertion, EKG and telemetry monitoring, tube feedings, wound care, suctioning, accu-checks, enemas, staple removal, and application of heat/cold therapy.
• Participate in discharge planning, patient and family education, and interdisciplinary team meetings.
• Document immunizations, medications, and patient care accurately.
• Provide emotional support to patients and families.
• Assist in mentoring and orienting new staff members.
• Demonstrate flexibility and resourcefulness in a fast-paced, dynamic environment.

Education and Licensure

Graduation from an accredited nursing program and current licensure as a Licensed Practical Nurse (LPN) eligible to practice in the Midwest region.

Experience and Skills

• Strong problem-solving and analytical abilities to quickly diagnose and resolve issues.
• Excellent verbal and written communication skills, with the ability to train and support others effectively.
• Knowledge of data protection regulations such as HIPAA and experience implementing patient data security measures.
• Attention to detail and commitment to maintaining data accuracy and patient privacy.
• Ability to prioritize workload efficiently in a fast-paced setting.
• Current Basic Life Support (BLS) certification by the American Heart Association.
