We are seeking a compassionate and skilled Registered Nurse (RN) Care Coordinator to join our healthcare team serving the Midwest region. This role focuses on delivering comprehensive, patient-centered care to individuals managing multiple chronic and preventive health conditions, aiming to enhance their overall well-being.

Key Responsibilities

• Provide holistic counseling and support to patients, addressing barriers to care and promoting medication adherence, healthy lifestyle choices, and self-management strategies.
• Manage care and follow-up for high-risk patients, utilizing monthly reports to monitor outcomes and coordinate with healthcare providers.
• Conduct nursing assessments and medical triage for acute issues and medication side effects, ensuring appropriate referrals and monitoring.
• Educate patients, families, staff, and community partners on chronic disease management and preventive screenings in an organized and effective manner.
• Facilitate access to support services by acting as a liaison with community agencies and connecting patients to financial assistance and entitlement programs.
• Oversee remote patient monitoring and chronic care management initiatives.
• Coordinate patient referrals and follow-ups with specialty medical, mental health, substance abuse, and community services.
• Interpret and communicate complex documents, reports, and regulations; prepare reports and present information to management and staff.
• Collaborate with physicians, healthcare providers, and external agencies to ensure continuity of care and treatment plans.

Education and Experience

• Graduate of an accredited nursing program with current RN licensure valid in the applicable state.
• Preferred minimum of two years’ experience as a registered nurse.

Requirements

• Proficient computer skills with the ability to learn new systems quickly.
• Current Basic Life Support (BLS) certification according to American Heart Association standards.
• Completion or willingness to complete Mandatory Reporter training within six months of hire.
• Compliance with all federal, state, and professional licensure and regulatory training requirements.
