Job Description: Physical Therapist – Aurora, Illinois

We are seeking a qualified and compassionate Physical Therapist to join our healthcare team in Aurora, Illinois. Position Summary
The Physical Therapist will evaluate and treat patients in either an inpatient or outpatient setting. The role involves developing and implementing individualized treatment plans based on patient needs, medical history, and functional goals. The Physical Therapist will work collaboratively with an interdisciplinary healthcare team to support optimal patient outcomes.

Key Responsibilities

• Perform comprehensive physical therapy evaluations and assessments.
• Develop, implement, and modify individualized treatment plans.
• Provide therapeutic interventions focused on mobility, strength, balance, and functional performance.
• Educate patients and caregivers on treatment goals, home exercise programs, and discharge planning.
• Document evaluations, treatment sessions, and progress notes in accordance with regulatory and facility guidelines.
• Participate in interdisciplinary rounds, care conferences, and care coordination activities.
• Adhere to all professional, ethical, and safety standards.

Qualifications

• Doctor of Physical Therapy (DPT) degree or equivalent from an accredited program.
• Current Physical Therapy licensure in the state of Illinois or eligibility for licensure.
• CPR certification preferred.
• Ability to treat patients with a variety of medical and functional conditions.
• Strong communication, organizational, and documentation skills.
