Your Role and Responsibilities

• Performing thorough patient evaluations, ordering necessary tests, and developing tailored care plans
• Diagnosing and managing acute, chronic, or episodic health issues collaboratively with physicians, or independently as qualification allows
• Providing education to patients and families regarding wellness, preventive care, and disease management
• Writing prescriptions and implementing treatments within your prescribed scope
• Engaging in patient follow-ups and managing chronic conditions
• Cooperating with interdisciplinary teams to deliver coordinated, comprehensive care
• Meticulously documenting all patient interactions in electronic medical records (EMR)

Qualifications and Certifications

• Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP) degree
• Completion of an accredited Nurse Practitioner training program

Necessary Certifications

• Valid Nurse Practitioner license in Illinois
• National certification as a Family Nurse Practitioner (FNP-C, AANP or ANCC) or appropriate equivalent
• Certification in Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS), or ability to obtain within 90 days
