Description

• Conduct thorough patient evaluations in inpatient or outpatient settings and develop individualized, goal-oriented treatment plans
• Deliver evidence-based physical therapy interventions targeting strength, mobility, balance, endurance, and functional independence
• Partner with a multidisciplinary care team — including physicians, nursing staff, case managers, and fellow rehab specialists — to drive optimal patient outcomes
• Provide clear, empathetic education to patients and their families regarding therapeutic objectives, home programming, and discharge readiness
• Maintain accurate, timely documentation of evaluations, treatment sessions, and patient progress per facility protocols and regulatory standards
• Actively participate in care team huddles, interdisciplinary rounds, and transition-of-care planning

Education

• Doctorate in Physical Therapy (DPT) or equivalent degree from an accredited institution

Certifications

• Active state licensure as a Physical Therapist in Illinois
• CPR certification preferred
• Specialty certifications in areas such as geriatrics, neurology, or orthopedics are welcomed but not required
