Description

• Conduct thorough evaluations across inpatient or outpatient settings and develop customized, goal-oriented treatment plans
• Deliver targeted physical therapy interventions addressing mobility restoration, strength development, balance training, and functional independence
• Partner alongside physicians, nursing staff, case managers, and fellow rehabilitation specialists to ensure cohesive, patient-centered care
• Guide patients and their support systems through therapeutic goals, safe discharge planning, and home exercise programming
• Maintain accurate and timely clinical documentation consistent with facility protocols and regulatory standards
• Engage actively in interdisciplinary rounds and care conferences to champion coordinated, whole-person recovery

Education

• Doctorate in Physical Therapy (DPT) or equivalent graduate-level degree from an accredited institution

Certifications

• Active state licensure as a Physical Therapist in Illinois
• CPR certification preferred
• Advanced or specialty certifications such as neurological rehab or geriatric care are welcomed but not mandatory

Skills

• Prior clinical experience in inpatient or outpatient settings is strongly preferred
