Description

• Conduct comprehensive patient evaluations across inpatient or outpatient environments and develop individualized, goal-driven treatment plans
• Deliver hands-on physical therapy interventions targeting mobility restoration, strength building, balance, and overall functional independence
• Partner with a multidisciplinary team including physicians, nursing staff, case managers, and fellow rehabilitation specialists to drive optimal outcomes
• Provide patient and family education around therapeutic goals, safe home exercise programs, and discharge preparation
• Maintain thorough, accurate documentation of evaluations, treatment sessions, and patient progress in alignment with facility and regulatory standards
• Engage actively in interdisciplinary rounds and care conferences to support coordinated, patient-centered care delivery

Education

• Doctorate in Physical Therapy (DPT) or equivalent qualifying degree from an accredited institution

Certifications

• Active state licensure as a Physical Therapist in good standing
• CPR certification preferred
• Advanced or specialty certifications such as neurological or geriatric rehab are welcomed but not required

Skills

• Prior clinical experience is strongly preferred
• Demonstrated ability to manage patients presenting with complex medical and mobility challenges
• Outstanding interpersonal and communication skills with a natural ability to collaborate across care teams
