Description

• Conduct thorough evaluations in inpatient or outpatient settings and build individualized, goal-oriented treatment plans
• Deliver evidence-based physical therapy interventions targeting mobility, functional strength, balance, and overall independence
• Work alongside physicians, nursing staff, case managers, and fellow rehabilitation professionals to drive optimal patient outcomes
• Guide patients and their loved ones through therapeutic goals, home exercise programs, and discharge planning
• Maintain accurate, timely documentation of assessments, treatments, and patient progress in alignment with facility and regulatory standards
• Engage actively in interdisciplinary rounds and care conferences to ensure a unified, patient-first approach

Education

• Doctorate in Physical Therapy (DPT) or equivalent degree from an accredited institution

Certifications

• Active state licensure as a Physical Therapist in Indiana
• CPR certification preferred
• Specialty certifications in areas such as neurology or geriatrics are welcomed, though not required

• Prior clinical experience is strongly preferred
