Role Responsibilities

• Deliver comprehensive primary care across all life stages, from infants and adolescents to adults and older adults.
• Diagnose and manage acute illnesses, chronic disease, preventive health, and behavioral health concerns with a patient-first mindset.
• Perform thorough physical examinations, order and interpret diagnostic studies, and design individualized care plans.
• Work closely with behavioral health specialists, care coordinators, and referral partners to ensure seamless, integrated care.
• Contribute actively to quality improvement initiatives designed to raise the bar on patient outcomes and team efficiency.
• Maintain thorough and accurate documentation through the practice's electronic medical record system.
• Provide both in-office and telehealth visits to meet patients where they are.

Education Requirements

• MD or DO degree from an accredited medical school.
• Successful completion of an accredited Family Medicine residency program.

Certifications

• Board Certification in Family Medicine required.
• Active Indiana medical license or eligibility to obtain one.
• BLS and ACLS certifications preferred.

Skills and Experience

• Sharp clinical instincts with a strong foundation in preventive and proactive care delivery.
