Description

• Perform comprehensive patient evaluations across inpatient and outpatient settings and develop individualized, goal-oriented treatment plans
• Deliver targeted physical therapy interventions addressing mobility deficits, functional limitations, balance impairments, and strength restoration
• Work hand-in-hand with physicians, nursing staff, case managers, and interdisciplinary rehab teams to deliver cohesive, coordinated care
• Guide patients and their families through therapeutic goals, home exercise programming, and discharge planning with clarity and compassion
• Maintain thorough, timely documentation of evaluations, interventions, and patient progress in alignment with facility protocols and regulatory standards
• Engage actively in care team rounds and interdisciplinary meetings to ensure seamless transitions across care settings

Education

• Doctorate in Physical Therapy (DPT) or equivalent degree from an accredited institution

Certifications

• Active state licensure as a Physical Therapist in Indiana
• CPR certification preferred
