The Clinical Liaison RN plays a vital role in managing patient referrals, conducting evaluations, and coordinating admissions to ensure seamless care delivery. This position involves collaborating with insurance providers, discharge planners, and multidisciplinary teams to facilitate safe and timely patient transitions. Responsibilities include conducting patient and family interviews to assess needs, receiving physician orders, triaging calls, and providing follow-up visits to support positive patient outcomes across all age groups. The role also encompasses data abstraction, quality reporting, and participation in performance improvement initiatives to maintain compliance with healthcare standards and regulations.

Education: Graduation from an accredited nursing program is required. A Bachelor of Science in Nursing (BSN) is preferred.

Experience: Several years of clinical nursing experience is necessary. Prior hospice or home health experience and background in performance improvement or clinical analysis are advantageous.

Requirements: Current RN licensure in the state of Kansas, Basic Life Support (BLS) certification within three months of hire, valid driver’s license, and auto insurance meeting state requirements. Proficiency with MS Office and strong communication skills are essential.
